GREEN = PASS
A score of 80 to 100 points and/or no more than one
MAJOR (4 point) Centers for Disease Control and
Prevention (CDC) risk factor violation which is
CORRECTED during inspection.

YELLOW = CONDITIONAL PASS
A score of 75 to 79 points and/or two or more MAJOR (4
point) CDC risk factor violations which are CORRECTED
during inspection.
A re-score inspection fee at the Department’s
hourly rate applies.
RED = CLOSED
A score of less than 75 points and/or one or more
MAJOR (4 point) CDC risk factor violation which is NOT
CORRECTED during inspection or any imminent health
hazard.
