The Alameda County Safe Drug Disposal Ordinance (Ordinance) requires
pharmaceutical producers to prepare and submit an annual written report describing
their activities to comply with Health and Safety Code Section 6.53.080 and the
Alameda County Safe Drug Disposal Regulations (Regulations) Section 15. Alameda
MED-Project LLC (Alameda MED-Project) submits this 2015 Alameda MED-Project
Annual Report (Report) on behalf of its participating producers and in compliance with
the Ordinance and Regulations.
The Alameda MED-Project Product Stewardship Plan (Plan) was approved by the
Alameda County, Department of Environmental Health (Alameda County DEH) on
February 25, 2015. Accordingly, this Report covers the time period of February 25, 2015
through December 31, 2015.
Alameda MED-Project began implementation of the Product Stewardship Program
(Program) after the Plan approval date.
In keeping with the Plan, Alameda MED-Project is implementing the Program in
phases. During 2015, priority was given to developing a sustainable foundation that
will allow the Program to expand and grow. This initial phase included establishing
new relationships with potential Collection Sites, planning and conducting a first-ever
take-back event, performing educational and outreach efforts, developing and
enhancing agreements with vendors, LEAs, and pharmacies, developing procedures
and informational materials, and monitoring performance to ensure continuous
improvement.
Kiosk Collection Sites will be established with interested law enforcement agencies and
pharmacies that meet all local, state, and federal requirements and subsequently sign
agreements with Alameda MED-Project. In addition, outreach and educational efforts
include a combination of printed materials (such as signs and brochures), online media
(such as the website), video and audio messaging (such as public service
announcements) and a call center.
The ultimate goal of the Program is to provide county residents with convenient, safe,
and accessible methods to dispose of Unwanted Medicine.
After approval of the Plan, agreements and contracts were finalized with vendors and
other Program participants. In addition, all LEA kiosk Collection Sites identified to

participate in the first year of the Program were contacted, and a take-back event date
and site were established. Several communication and educational tools were put in
place, including the website and toolkit.
During March of 2015, a point of contact was established at each identified law
enforcement agency, and agreements and contracts for kiosk placement and take-back
events were distributed in December 2015. Alameda MED-Project continues to
communicate with each Collection Site Host, answer questions, gain feedback, and
further improve outreach materials, agreements, procedures, and the overall Program
design.
The Program is continuously monitored and adjustments are made to streamline and
simplify processes, as described in the rest of this report. This includes refining
informational and educational materials, procedures, and protocols. These efforts
improve the Program’s sustainability, and its ability to expand to meet goals effectively
and efficiently.
2. REPORTING ON GOALS IN 2015

2.1. Collection Goals
Short-term collection goals, as outlined in the approved Plan, included the placement of
approximately 20 kiosks in LEAs, with a goal of installing three kiosks within the first
90 days of implementation, and 12 take-back events, with a goal of conducting the first
take-back event within the first 30 days of implementation.
In 2015, these goals were not met due to unanticipated delays associated with the
establishing of agreements with law enforcement for kiosk collection and take-back
events.
Agreements to support take-back events were drafted for LEAs shortly after the Plan
was approved. Initial agreements were then distributed to the San Leandro and Union
City Police Departments in March 2015 in anticipation of a household hazardous waste
(HHW) event in late March and a recycling event in early April. Alameda MED-Project
was then directed to the Alameda County Sheriff’s Office (ACSO) by both agencies and
the agreements were subsequently sent to the ACSO for further review in early April
2015. In April, Alameda MED-Project put take-back agreements with other agencies on
hold in order to finalize the agreement with the ACSO, and to establish a standardized
approach for agreements going forward.
After several discussions with the ACSO and Alameda County Counsel (County
Counsel), the take-back agreement was put on the agenda for review by the Alameda

County Board of Supervisors in July 2015. The ACSO take-back agreement was
subsequently approved by the Board of Supervisors on July 28, 2015 (see Appendix A),
and the first take-back event was conducted on August 8, 2015.
Following the first event, procedural issues were raised by the ACSO to the Alameda
MED-Project, and participation in events was further delayed in order to address these
issues. Discussions with the ACSO and County Counsel from early September through
mid-November 2015 resulted in updates to take-back event operating procedures that
will streamline and standardize future events. For more information about this topic,
please refer to Section 6.
A kiosk collection agreement was also distributed to the ACSO and County Counsel for
review in early April 2015. After reaching agreement on the approach to take-back
events in November 2015, County Counsel resumed its review of the kiosk collection
agreement in late 2015.
Based on past experience and challenges associated with finalizing agreements,
Alameda MED-Project has adjusted this process in several ways. In order to streamline
and accelerate the agreement review process, Alameda MED-Project worked closely
with the ACSO to create a template agreement for take-back events that addresses all
concerns raised to date. The new template agreement includes additional clarity
regarding the handling of materials at events, and reduces the number of review cycles
by using one agreement for multiple future events. Kiosk agreements were similarly
updated and refined.
Collection Sites and LEAs vary widely in their process of agreement review, leading to
unpredictable durations and lead times to gain signatures and completed agreements.
In an effort to streamline this process and reduce review cycle times, Alameda MED-
Project created email templates for frequently asked questions related to kiosk
specifications, take-back events, and agreements, and distributes this information to
sites upon request.
Alameda MED-Project plans to double take-back efforts in 2016 and conduct 24 or more
take-back events by hosting events at interested Collection Sites, collaborating with
Household Hazardous Waste events, and participating at larger fairs and festivals. For
more information about this effort, please see Section 3.
Alameda MED-Project intends to increase kiosk placement in 2016 by working with all
LEAs that express interest in participating in the Program, and by reaching out to all
licensed pharmacies and health systems to assess interest in participating as a
Collection Site, with the goal of establishing approximately 100 kiosks by the end of
2017. For more information about this effort, please see Sections 3 and 5.

2.2. Education and Public Outreach Goals
Short-term education and public outreach goals, as outlined in the approved Plan,
included the development of baseline statistics for the call center and website, the
number of media outlets receiving a press advisory, the percentage of community
centers reached, and the number of messages to MED-Project returned within a pre-
determined time frame.
MED-Project generated these baseline statistics in 2015. Call volume to the call center at
baseline was less than five calls per month, with a goal of increasing call volume as the
volume of events and kiosk placements increase. All callers that left a voice message
were contacted by the call center by the following business day.
Website statistics at baseline included a total of 80 new users spending an average of 2.5
minutes on the website, viewing an average of three web pages per visit. By increasing
outreach through distribution of brochures and surveys by mail, the placement of
brochures and signage at Collection Sites, and advertising prior to take-back events,
Alameda MED-Project expects to increase online traffic.
The Alameda MED-Project website (www.med-project.org/alameda) went live on
March 27, 2015 and includes information about the Program, the text of public service
announcements (PSAs), a set of FAQs, a printable brochure, and a tool residents can use
to locate take-back events and kiosk Collection Sites. Website page views and unique
visitor information captured during this phase will be used to establish a baseline by
which usage can be measured as the number of events and Collection Sites increases.
The radio and TV PSAs were posted on the website in August 2015, and were available
to all members of the community for viewing, download, and reuse. The PSAs can be
adjusted and used for radio and TV spots based on the requirements of each media
outlet.
The toll-free number for the call center was available within the first 30 days after the
Plan was approved, and is available in both English and Spanish. Messages left on the
voicemail are returned within 24 hours per the approved Plan.
LEAs were contacted by phone or email, and brochures were distributed by mail in
April 2015 to inform these potential Collection Sites of the new Program (see Table 1 for
a listing of LEAs that were contacted). Brochures continue to be distributed to
pharmacies, health systems, and community organizations in order to broaden
outreach, increase call center and website statistics, and develop interest in participation
as a Collection Site. For more information regarding outreach, see Sections 3 and 5.
Recent feedback about the website and call center raised concerns regarding download
and routing issues, which were subsequently corrected. As a result of this feedback, the
website now undergoes weekly quality assurance testing to ensure navigation, content,

links, and downloads are functioning properly, and the call center also undergoes
weekly quality assurance testing to ensure scripts, navigation, call routing, and call
attendants are following procedures as outlined in the Plan.
Alameda MED-Project will continue to review performance and revise processes,
systems, and agreements as needed to improve the infrastructure, quality, efficiency,
and sustainability of the Program, and to ensure alignment with county, state, and
federal laws and requirements.
2.3. Regulatory Outreach Goals
Short-term regulatory outreach goals, as outlined in the approved Plan, included
contacting members of the classes of potential collector entities identified, inviting them
to participate in the Plan based on relevant template agreements, and setting targets
based on identities of classes of collectors.
As described in Sections 2 and 5, Alameda MED-Project distributed brochures to all
targeted LEAs in April 2015. Section 3 describes Alameda MED-Project’s distribution of
brochures promoting the Plan to pharmacies and public health facilities in February
2016. Pharmacies also received surveys at that time. As described in Section 2.1, take-
back event contracts were distributed to LEAs in March and April 2015. LEA and
pharmacy kiosk collection site agreements were distributed in December 2015 and
again in February 2016 to comply with the revised Ordinance..
2.4. Take-Back Events

The first take-back event facilitated by the Alameda MED-Project was hosted in Castro
Valley at Canyon Middle School (19600 Cull Canyon Road) from 8:00 AM to 2:00 PM on
August 8, 2015, where approximately 289 residents dropped off 336.2 pounds of
Unwanted Medicine. The take-back event was part of a larger recycling event where
other materials were collected separately. Advertising for the event is shown in the
flyer below, and was performed by the Castro Valley Sanitary District.

As previously described, procedural concerns at the event were identified by the ACSO,
and communicated to the Alameda MED-Project. As a result, Alameda MED-Project
held a meeting with the ACSO and County Counsel on September 3, 2015 to discuss
policies and procedures. After further review, Alameda MED-Project, ACSO, and
County Counsel came to agreement on November 16, 2015 about procedures going
forward. The take-back event agreement and operating procedures were then modified
to reflect the outcome of these discussions, and are continually refined to ensure
alignment with the Ordinance, Regulations, the Plan, and other contracts and
agreements. For more information, see Sections 4 and 6 of this report.
2.5. The Collection System

The Alameda MED-Project safe drug disposal collection system places emphasis on
Collection Sites at LEAs and pharmacies to best serve county residents. This is
complicated by the broad geography and diverse demographics of Alameda County. In
April 2015, approximately 20 law enforcement agency sites (Table 1) were contacted by
phone as potential Collection Sites, with the aim of improving access for all county
residents.

In December 2015, kiosk collection agreements were distributed to law enforcement
agencies in 23 locations. The location of each site and the status of the agreements are
outlined in the table below.
Agreements that are ‘pending review’ include sites that have not reviewed the
documents to date or may not be able to host kiosks. Agreements that are under
‘review’ include sites that have acknowledged receipt and are in the process of
reviewing the document. Agreements that are under ‘final review’ include sites that
have reviewed the documents and provided information to finalize the agreement.

3.1. Take-Back Events
Alameda MED-Project is exploring opportunities with several Alameda County health
systems in 2016, including Eden Hospital, Washington Hospital Health System (WHHS)
and the Alameda Health System (AHS), to conduct one-day take-back events in
multiple locations where residents can drop off their Unwanted Medicine. Such events
would be held indoors, eliminating problems associated with bad weather, and would
enable Alameda MED-Project to promote multi-site take-back days across the county.
Conducting indoor multi-site one-day take-back events (which can be offered at any
time of the year) would enhance the Program's ability to reach the community more
broadly through multiple media channels including radio, television, online, and
though printed material. The sites targeted for upcoming one-day multi-site take-back
events include:

Take-back agreements have been updated to reflect changes to the Ordinance as
approved by the Board of Supervisors on February 2, 20162, and the first of these
agreements was issued to the ACSO on February 23, 2016. The agreement is on the
March 1. 2016 agenda for the Alameda County Board of Supervisors.3
3.2. Kiosk Collection Sites
Alameda MED-Project distributed contracts for LEA kiosk collection sites in December
2015. With changes to the Ordinance that were approved by the Alameda County
Board of Supervisors on February 2, 2016, updates to kiosk agreements were also
required and the agreements were reissued to 20 LEAs on February 25, 2016, and to six
interested pharmacies on March 1, 2016. Priority will be given to interested LEAs and
pharmacies, as well as to qualifying sites that currently participate in safe drug disposal
programs.4 In 2016 and 2017, Alameda MED-Project will target placement of 100 ore
more kiosks at qualifying and interested LEAs, pharmacies, and health systems across
Alameda County to offer convenient disposal methods for county residents.
3.3. Education and Public Outreach
Alameda MED-Project reached out to pharmacies again in early 2016, issuing outreach
letters, surveys, and brochures by mail on February 19, 2016 to determine their interest
in participating in the Program as Collection Sites (see Appendix B). The pharmacies
that received educational materials, an outreach letter, and a participation survey
questionnaire are listed in Appendix C. Site assessments will be conducted by phone
and/or in-person for pharmacies that express interest in participating in the Program.
A Fact Sheet describing kiosk specifications is under development and will be
distributed to all interested Collection Sites beginning in March 2016.
Outreach materials will be provided to the following organizations in March 2016 to
further extend community education, and to pursue one-day take-back events and
kiosk placement at qualifying locations.

A Fact Sheet describing take-back event operations and logistics, along with a brief
description of roles and responsibilities, is under development and will be distributed
to all interested Collection Site Hosts in March 2016.
Social media websites for the Alameda MED-Project, including Facebook and Twitter,
will be released in March 2016 to increase online access for Alameda County residents.
And in early April 2016, news and press releases will be released prior to the one-day
multi-site take-back event through radio, television and online venues. Events and
kiosk information will be posted to the www.med-project.org/alameda website as they
are scheduled and confirmed.

Alameda MED-Project will continue to monitor the website and call center to assure
quality, and to assess the impact of outreach and education efforts. The data collected
will be compared to baseline data as part of the Alameda MED-Project’s commitment to
continuous improvement.
4. COLLECTION PRACTICES

Available documents appear to indicate that Stericycle collected 336.2 pounds of
material at the August 8, 2015 take-back event and facilitated the destruction of 328
pounds of this material at the Clean Harbors Aragonite hazardous waste disposal
facility on February 23, 2016. These apparent deviations are within a reasonable margin
of error. MED-Project will supplement this Report with any relevant additional
information it identifies regarding the collection, transport, and disposal of material
from the event.
Public advertising surrounding the event indicated that controlled substances would
not be accepted. To the extent that any controlled substances were collected, however,
the transportation and delay in disposal of the collected materials would not meet
MED-Project’s requirement that all collection, transport, and disposal requirements of
the DEA Rule be met by its vendors. In addition to any counter measures identified by
the investigation and to ensure collection and disposal methods meet all applicable
legal requirements, MED-Project will, in conjunction with changes necessitated by the
recently adopted modifications to the Alameda County Ordinance, which expand the
type of materials regulated under the Ordinance, revisit all agreements and standard
operating procedures with Stericycle.
Participation in the Program by vendors and Collection Sites requires executed
agreements. In addition, policies and procedures are in place to ensure that collection,
transport, and disposal of Unwanted Medicine are conducted according to the
approved Plan.
Each site that volunteers to participate in Unwanted Medicine collection by hosting a
kiosk is required to sign a kiosk collection agreement prior to installation.

Alameda MED-Project uses a combination of printed materials, online media outlets,

awareness of the Program, and to educate county residents about safe and available
methods for disposal of Unwanted Medicine.
In April 2015, brochures (Figure 3) were distributed to all targeted LEA Collection Sites
(Table 1), and to pharmacies (Appendix C) and health systems (Section 3.3) in February

A call center that provides information about drug disposal options in English and
Spanish is also available for Alameda County residents, businesses, organizations, and
agencies at (844) MED-PROJ.
Outreach activities are evaluated by capturing and analyzing data from the website and
call center and comparing to baseline. During upcoming outreach campaigns, statistics
will be compared against baseline to assess online and call center activity. For

No safety or security incidents were identified during the collection, transport, or
disposal of Unwanted Medicine during the reporting period.
However, ACSO raised procedural issues regarding take-back event collection with
Alameda MED-Project after the August 8, 2015 event. These involved a dispute over
the interpretation of the take-back agreement between Alameda MED-Project and
ACSO regarding Alameda MED-Project’s ability to screen certain materials from
collection that did not meet the definition of a Covered Drug (i.e., non-prescription
drugs) under Alameda Health & Safety Code, Chapter 6.53.030(3). After the August 8,
2015 take-back event, ACSO informed MED-Project that the screening of these materials
potentially led to what ACSO perceived as an increased risk of diversion. No safety or
security incident was reported. Alameda MED-Project and the ACSO, through
Alameda County Counsel, agreed to end the screening and denial of any non-
prescription drugs at take-back events to address ACSO’s concerns. Alameda County
has since revised the Ordinance, and an amended take-back agreement reflecting these
revisions was subsequently signed by Alameda MED-Project on February 23, 2016. This
agreement was sent to Alameda County Counsel for processing.
Safety and security is paramount for the Alameda MED-Project Program. Thus, specific
language is provided in all agreements with vendors and Collection Sites to emphasize
this priority and prevent diversion.
