Alameda County Safe Drug Disposal Ordinance (“Ordinance”) Section 6.53.080 and Alameda
County Safe Drug Disposal Regulations (“Regulations”) Section 15 requires MED-Project LLC
(“MED-Project”) to prepare and submit an annual written report describing activities between
January 1, 2018 and December 31, 2018. MED-Project submits this 2018 Alameda County,
California MED-Project Annual Report (“Report”) on behalf of its participating producers and in
compliance with the Ordinance and Regulations.
The Alameda MED-Project Product Stewardship Plan (“Plan”) was approved by the Alameda
County, Department of Environmental Health (“Alameda County DEH”) on February 25, 2015.
In keeping with the Plan, MED-Project is implementing the Product Stewardship Program
(“Program”) in phases. During 2018, priority was given to establishing additional kiosk drop-off
sites throughout Alameda County (“County”), conducting take-back events in alignment with
approved Plan goals, and performing outreach to eligible pharmacies and Law Enforcement
Agencies (“LEAs”) throughout the County. As a result of these efforts, MED-Project’s Program
collected and disposed of Unwanted Medicine from seventy (70) kiosk drop-off sites (seventy-
one (71) kiosks) and twelve (12) take-back events in 2018.
Kiosks were activated in pharmacies and LEAs that meet all local, state, and federal
requirements and that executed agreements with MED-Project prior to kiosk installation and
activation. In addition, outreach and educational materials were distributed to all interested and
participating sites, including signage and/or brochures.
As outlined in the approved Plan, the ultimate goal of the Program is to provide County residents
with convenient, safe, and accessible methods to dispose of Unwanted Medicine. MED-Project
anticipates a steady increase in kiosk drop-off sites throughout 2019 as a result of Program
activities in 2018. MED-Project continues to communicate with kiosk drop-off site hosts to
distribute outreach materials, review procedures as needed, and improve overall Program design.
MED-Project submitted revision requests and a revised plan in 2018 to address both
improvements in collection efforts and changes in program implementation, such as changes to
kiosk signage to address the California Board of Pharmacy Prescription Drug Take-
Back Services Regulations, Cal. Code Regs. tit. 16, §§ 1776 et seq. (2017) (the “Board of
Pharmacy Regulations”)1.

2.1. Collection Goals
Long-term collection goals, as outlined in the approved Plan, include the expansion of kiosk
placement to additional eligible LEAs and pharmacies with a goal of establishing up to one
hundred (100) kiosk drop-off sites, assuming viable locations are available. Program goals set an
ongoing target of twelve (12) take-back events per year. MED-Project held twelve (12) take-
back events in 2018.
In 2018, law enforcement oversight of events was performed by the Alameda County Sheriff’s
Office (“ACSO”) for all twelve (12) of the events. See Table 1 for more details.

MED-Project plans to conduct up to twelve (12) take-back events in 2019 by collaborating with
Alameda County Household Hazardous Waste (“HHW”) drive-through events and LEAs. For
more information about this effort, please see Section 3.
In 2018, agreements to host kiosk collection services were executed with two (2) LEAs, thirty-
three (33) pharmacies, and three (3) healthcare organizations, consisting of a total of thirty-eight
(38) kiosk drop-off sites. Kiosks were subsequently installed in thirty-eight (38) sites and
became available to the public, providing a total of seventy (70) active kiosk-drop off sites
(seventy-one (71) kiosks) in 2018. See Table 2 for more details.

MED-Project intends to increase the number of active kiosk drop-off sites in 2019 by continuing
to work with eligible LEAs and pharmacies that express interest in participating in the Program.
Although MED-Project has not established up to one hundred (100) kiosk drop-off sites, it
anticipates adding new kiosk drop-off sites in 2019 as more options for collection services are
incorporated into the approved Plan and Program. For more information about this effort, please
see Section 3 and Appendix A.

2.2. Education and Public Outreach Goals
MED-Project conducted twelve (12) take-back events, performed installations at thirty-eight (38)
kiosk drop-off sites, performed print advertisements, and distributed materials, including
brochures and posters, to all seventy (70) active kiosk drop-off sites in 2018. The toll-free
number for the call center continues to be available in both English and Spanish. The MED-
Project call center was converted to a 24-hours-per-day by seven-day-per-week (24 by 7)
schedule in 2018 to accommodate for calls from residents during non-business hours.
In an effort to increase website visits, MED-Project continued outreach in 2018 through social
media websites such as Twitter and Facebook. Brochures and materials were distributed to
nearby pharmacies, LEAs, clinics, hospitals, and community groups, and other media outlets
were contacted to increase outreach prior to events. Website statistics for 2018 can be viewed in
Table 3.

The MED-Project website is regularly updated to ensure alignment with Program activities. The
website provides contact information that County residents, kiosk drop-off site hosts, and
community agencies can use to obtain Program information.
Information regarding kiosk drop-off sites is available in both map and list formats at

The Plan’s long-term education and public outreach goals provide that MED-Project may revise
and/or add communication materials based on changes to the Plan, and that MED-Project will
review media and public outreach and collect feedback to improve operations. The methodology
employed by MED-Project utilizes qualitative and quantitative techniques to evaluate outreach
and promotional activities, including unstructured interviews, data collection at take-back events,
and website statistics. MED-Project is also incorporating information collected, best practices,
and lessons learned from other jurisdictions to improve its Program.
MED-Project performed unstructured interviews of residents, potential and existing kiosk drop-
off site hosts, stakeholders, health systems, and local agencies in various settings. The purpose
of these interviews was to elicit feedback regarding Program awareness and activities.
Information was gathered during take-back events, site visits, and regularly scheduled meetings.

Data collection was performed during and after take-back events to capture attendance,
collection, transport, and Unwanted Medicine disposal statistics. Website visits were increased
compared to the previous calendar year.
Through its data collection, MED-Project learned that events held in combination with the
County’s HHW events showed increased consumer participation as compared to events hosted at
fairs or festivals. While MED-Project advertised its events extensively on social media and the
MED-Project website, consumer participation at events was highest when held in combination
with HHW events. Events at fairs and festivals collected fewer than 63 pounds of Unwanted
Medicine per event on average, while events that were promoted jointly with HHW collected
more than 333 pounds of Unwanted Medicine per event on average.
As a result of these lessons learned, MED-Project may be able to improve collections at 2019
events by focusing mainly on conducting events in conjunction with planned HHW events.
MED-Project is working in collaboration with Alameda County DEH to explore conducting
Unwanted Medicine take-back events at all Alameda County HHW events in 2019. MED-
Project provides media release templates to host sites, which enable sites to promote events in
combination with MED-Project’s efforts.
Regarding kiosk drop-off sites, MED-Project will continue to publicize these locations in 2019,
and will provide each site with posters, brochures, and media release templates. Upon activation,
all existing and new kiosk drop-off sites are promoted on social media and the MED-Project
website. Fliers for residents are also produced to increase public awareness.
Efforts to recruit additional health systems and chain pharmacy kiosk drop-off sites in 2019 may
also increase awareness and allow MED-Project to implement numerous additional kiosk drop-
off sites throughout Alameda County.
MED-Project continues to increase awareness of its Program in Alameda County and to recruit
additional kiosk drop-off sites by performing site visits and distributing outreach materials to
potential kiosk drop-off sites. MED-Project is also distributing materials to health associations,
systems, and organizations, as described in Appendix E of the approved Plan.

Over 2,643 pounds of Unwanted Medicine were collected at the 2018 events and subsequently
incinerated. MED-Project collaborated with ACSO and held events at twelve (12) different
venues. Seven (7) of the events were in collaboration with County HHW drive-through events,
and five (5) of the events were conducted at festivals or fairs.

The MED-Project safe drug disposal collection system places emphasis on kiosk drop-off sites at
LEAs and pharmacies to best serve County residents. In 2018, additional kiosks were installed
and became available to the public at thirty-eight (38) locations throughout the County,
providing a total of seventy (70) kiosk drop-off sites and seventy-one (71) kiosks available to
County residents. Please see Figure 1 and Table 2 for more information regarding active kiosk
drop-off site locations in Alameda County in 2018.

Updated signage describes products that are accepted and not accepted in kiosks, with additional
information directing residents toward disposal options for items that are not accepted in kiosks.

3.1. Take-Back Events
MED-Project is working closely with Alameda County DEH to continue to conduct take-back
events in collaboration with HHW drive-through events throughout the County in 2019. In
addition, MED-Project will respond to requests to conduct take-back events in collaboration with
LEAs, upon request. Take-back event services agreements will continue to be issued to LEAs
that are interested in overseeing take-back events.
3.2. Kiosk Drop-Off Sites
MED-Project continues to work closely with LEAs, pharmacies, and hospitals/clinics with on-
site pharmacies that are interested in becoming kiosk drop-off sites. Agreements have been
distributed to ten (10) pharmacies for review. See Table 5 for more information. MED-Project
continues to schedule kiosk installation and activation at eligible sites with executed agreements,
with the goal of establishing up to 100 kiosks in Alameda County. Pharmacies that are targeted
as potential kiosk drop-off sites via outreach through periodic calls, emails, and site visits, as are
listed in Appendix A.
3.3. Education and Public Outreach
MED-Project supports kiosk drop-off sites with printed materials, including brochures and
posters, as well as posted information on the website.
Outreach materials, including brochures and fact sheets, will continue to be provided to sites
interested in participating in the Program, as well as selected organizations, in order to further
educate the community and pursue kiosk placement at eligible locations.

Social media websites for MED-Project, including Facebook and Twitter, will continue to post
news and events for Alameda County residents. Events and kiosk information will also be
posted to the MED-Project website as they are confirmed and available to the public. MED-
Project will run three campaigns throughout 2019 on multiple media platforms including print,
radio, television and social media platforms, to further increase awareness of its Program.
In an effort to increase awareness of its Program and further evaluate effectiveness, MED-Project
also proposes to conduct a biennial survey of residents and health care providers in the County,
in conjunction with the survey conducted under MED-project’s stewardship program for the
collection of sharps from households, in the revisions to the Updated Plan submitted to Alameda
County DEH for review on February 15, 2019 (“Updated Plan”).

The MED-Project call center was converted to a 24-by-7 schedule in 2018, and MED-Project
proposes to add Chinese, Vietnamese, and Tagalog support to the call center, as well as other
outreach and education materials, to increase services to Residents, in the Updated Plan.
4. COLLECTION PRACTICES

MED-Project provided County residents a combination of take-back events and kiosk drop-off
sites for the collection of Unwanted Medicine in 2018. Policies and procedures for collecting,
transporting, and disposing of Unwanted Medicine, as described in the approved Plan, were
followed during the reporting period, with exceptions as described below and in Appendix D.
Take-back events were conducted from January through October with events hosted at 11
separate venues. Over 2,643 pounds in total were collected during 2018, with an average of
approximately 220 pounds collected per event. Oversight was performed by ACSO at all events
in 2018.
MED-Project submitted requests for exceptions to utilize common carrier for the transportation
and disposal of Unwanted Medicine, which were granted by Alameda County DEH. Take-back
events in 2018 utilized common carrier transport and were disposed of at Heritage Thermal
Services in Ohio.
Kiosk services agreements were executed with a total of two (2) LEAs and thirty-three (33)
pharmacies and three (3) healthcare organizations, representing a total of thirty-eight (38) kiosk
drop-off sites. Kiosk drop-off sites were implemented at LEAs and pharmacies in 2018 with a
total of thirty-nine (39) kiosk installations (one site has two (2) kiosks) and activations at two (2)
LEAs and thirty-six (36) pharmacies and/or hospitals/clinics with an onsite pharmacy.
As reported in the 2017 MED-Project Alameda County, California Annual Report, based upon
the potential for loose Unwanted Medicine to accumulate in the drop-slot, MED-Project worked
with the kiosk vendor to design a retrofit for the drop-slot chute. MED-Project completed the
retrofit for all kiosks placed under the Plan in Alameda County on March 1st, 2018. A total of
thirty-two (32) drop-slot chute retrofits were performed. All new kiosk installations were
equipped with this retrofit.
MED-Project continues to capture data from take-back events and kiosk drop-off sites to
evaluate performance on a routine basis.

MED-Project uses a combination of printed materials, online media outlets, public service
announcements, and other methods, such as the call center, to increase awareness of the
Program, and to educate County residents about safe and available methods for disposal of
Unwanted Medicine. In 2018, brochures (Figure 6) and posters were distributed to all active
kiosk drop-off sites (Table 2), to ensure materials and information are available for residents.
A primary objective of outreach efforts in 2018 was to raise awareness of the Program.
Materials will continue to be distributed to active kiosk drop-off sites, community organizations,
agencies, associations, and health systems to increase awareness and recruit additional sites for
potential kiosk placement.
In 2017, MED-Project selected a vendor to produce a 30-second animation that is now utilized as
a Public Service Announcement for residents regarding safe and secure methods of disposal of
Unwanted Medicine. MED-Project continues to utilize the animation video on social media
websites and other media platforms, such as television. See Appendix C for more information
regarding outreach activities in 2018.

Event: Notice of Change in Disposal Facility for Unwanted Medicine Boxes and Liners
Date: November 7 and 12, 2018
Description:
MED-Project notified the Department that kiosk boxes and inner liners containing unwanted
medicine (“Unwanted Medicine Liners”) managed as part of the Plan would need to be destroyed
at a back-up destruction facility because of cancelled appointments at the primary facility,
Heritage Thermal Services, located at 1250 Saint George Street in East Liverpool, Ohio.
Unwanted Medicine Liners were destroyed at the Veolia -Port Arthur permitted hazardous waste
facility located at 7665 Texas Highway 73, Beaumont, Texas 77705 (“Veolia - Port Arthur
Facility”) on November 7, 2018, and also at the Stericycle United States Drug Enforcement
Administration (“DEA”) registered collector medical waste incinerator located at 1901 Pine Ave
SE, Warren, Ohio 44483 (“Stericycle, Inc. Warren Ohio Facility”) on November 12, 2018,
because of scheduling and logistical issues at the Veolia - Port Arthur Facility.
• 58 Unwanted Medicine Liners were destroyed at the Veolia - Port Arthur Facility; and
• 29 Unwanted Medicine Liners were destroyed at the Stericycle, Inc. Warren Ohio
Facility
Vendor reported that the action to send Unwanted Medicine Liners to the Veolia - Port Arthur
Facility and the Stericycle, Inc. Warren Ohio Facility was due to destruction appointment
cancellations at Heritage Thermal Services—Liverpool in October 2018. In some cases, these
cancellations delayed the destruction of Unwanted Medicine Liners beyond what MED-Project
policies require. Vendor reported that Heritage Thermal Services-Liverpool could not provide a
confirmed destruction appointment before November 16, 2018, making it necessary to send
Unwanted Medicine Liners to the Veolia - Port Arthur Facility and the Stericycle, Inc. Warren
Ohio Facility for destruction as soon as possible.
Corrective Action:
Upon being notified of these issues by Vendor, MED-Project immediately; (1) confirmed with
Vendor that Unwanted Medicine Liners remained securely stored; (2) requested the complete
inventory of impacted Unwanted Medicine Liners and retention dates; and (3) approved Vendor
proposed plans to destroy Unwanted Medicine Liners as soon as practicable at either the Veolia-
Port Arthur Facility, or at the Stericycle, Inc. Warren Ohio Facility, if the Veolia - Port Arthur
Facility was unavailable.
Stericycle notified MED-Project that capacity limits was a contributing factor to the destruction
appointment cancellation and that the Heritage Thermal Services-Liverpool facility had reached
capacity for taking Unwanted Medicine Liners. MED-Project requested Department approval
for additional disposal facilities in the revised Product Stewardship Plan for Unwanted Medicine
from Households submitted on February 15, 2019.

MED-Project was notified on November 30, 2018 that a Heritage Thermal Services December
destruction appointment was cancelled. As a result of the cancellation, Unwanted Medicine
Liners managed as part of the Plan needed to be destroyed at a back-up destruction facility.
Unwanted Medicine Liners were destroyed at the Veolia - Port Arthur Facility on December 11,
2018.
• 31 Unwanted Medicine Liners were destroyed at Veolia-Port Arthur Facility on
December 11, 2018.
Vendor reported that the action to send Unwanted Medicine Liners to the Veolia-Port Arthur
Facility was due to destruction appointment cancellations at Heritage Thermal Services in
December 2018.
Corrective Action:
Upon being notified of these issues by Vendor, MED-Project immediately: (1) confirmed with
Vendor that Unwanted Medicine Liners remained securely stored; (2) requested the complete

Event: New Model Kiosk Installations
Date: September 2018
Description:
The Department inspected a new kiosk prototype design at an in-person meeting on July 26,
2018 and, subsequently, these kiosks were ordered and installed at eight kiosk drop-off sites in
September 2018. The new kiosks have several improvements, including removal of indicator
lights. Removing the indicator lights alleviates the burden on kiosk drop-off sites to charge
indicator light batteries and, based on MED-Project and vendor experience managing collection
frequencies, does not affect Collection Site convenience.
Corrective Action:
MED-Project reviewed change procedures with staff and submitted a written change request for
the new kiosk prototype design on January 17, 2019 (revised and submitted on February 23,
2019).
Event: Unwanted Medicine Kiosk Service Procedure
Date: August 14, 2018
Description:
Overfilled Unwanted Medicine inner liner was serviced by transferring the overflow contents to
a second Unwanted Medicine box and inner liner. The original Unwanted Medicine inner liner
was sealed and packaged for transport. The second partially filled Unwanted Medicine box and
inner liner were placed back into the Unwanted Medicine kiosk.
Corrective Action:
MED-Project notified vendor of the discrepancy and directed vendor to take corrective action.
MED-Project monitored performance with no recurrence reported in 2018.

The number of times each kiosk drop-off site was serviced for pick-up and the total pounds
collected and disposed of from take-back events and kiosk drop-off sites in Alameda County
during 2018 is provided below. Because the disposal of Unwanted Medicine occurs several days
after pick-up at a kiosk drop-off site, disposal of some Unwanted Medicine collected in late 2018
did not occur until early 2019 and is not reflected in the disposal figures below. Unwanted
Medicine collected at kiosk drop-off sites in 2018 was incinerated at Heritage Thermal Services
– Ohio, Veolia – Port Arthur, and Stericycle, Inc., Warren, Ohio disposal facilities.
All Unwanted Medicine collected at Take-Back Events was incinerated at Heritage Thermal
Services – Ohio.
