Re: Exelixis'Transition to the Alameda Med-Project LLC Product Stewardship Plan
Dear Mr. Seery,
On April 4,2OL6, Exelixis notified Alameda County of our intention to retire our approved Safe Drug Disposal
Program Plan and join the approved Alameda Med-Project LLC Product Stewardship Plan.
Exelixis submitted the applicable forms to the Pharmaceutical Product Stewardship Work Group ("PPSWG")
on March tL, 2OL6 and we received our formal notification of acceptance on April 25, 20L6, which
confirmed our membership with the PPSWG and the Alameda Med-Project LLC Product Stewardship Plan.
From January L,2OL6 through the end of April, Exelixis fully maintained the maíl-back option for all of our
patients seeking to return their unused product under our existing plan with Alameda County. As of April
30,20t6, under our Plan, Exelixis provided zero (0) mail-back containers to patients within Alameda County.
Since no patients within Alameda County utilized the mail-back disposal option under our approved Plan,
our third party logistics company processed zero (0) mail-back containers for destruction during this time
period.
We hope our letter sufficiently addresses any remaining questions that Alameda County may have related to
the retirement of our Exelixis approved plan and the transition to the Alameda Med-Project LLC Product
