Directions: Please complete all fields with the requested information. Signatures of both the “TFSO
Registrant” and the food facility “Permit Holder” are required. Submit this completed form, the TFSO
operations plan and the TFSO Registration Fee. This Annual Registration Fee will cover the evaluation of
the TFSO Operations Plan for this food facility only plus the onsite facility evaluation inspection. An
additional fee would be required to assess each additional food facility as a proposed Permitted Food
Facility. Once the Registration form and Operations Plan has been reviewed, the TFSO Registrant will be
contacted to arrange for the onsite inspection. Upon approval, a Registration Certificate will be issued
and must be available onsite when operating the TFSO.
