What has stayed consistent? 4 Home Together 2026 •Prevent homelessness •Connect to shelter and resources •Increase housing solutions •Strengthen coordination, communication, and capacity Home Together 2030 •Prevention •Access •Shelter •Housing •Health Care •System Foundations 5 Home Together 2030 is a refresh of Home Together 2026 Since the launch of the HT2026 Community Plan much has been learned, and political, economic and community landscapes have changed. 6 HT2030 envisions a system that moves people from crisis to stable housing quickly and for the long term •It is a framework for coordinated, strategic action – not a spending plan or implementation handbook •It focuses on the homelessness response system, while acknowledging other systems and economic and political forces that impact the homelessness response system’s success •It sets goals that are achievable with sustained local investment, stable federal and state partnership, coordination across jurisdictions, and balanced implementation across pillars ➢Reduce overall homelessness by one-third ➢Reduce unsheltered homelessness by 50% Pillar 1: Prevention What objectives and strategies are included in the Prevention pillar? 7 8 Prevention HT2030 is organized around five interdependent pillars, supported by cross-cutting system foundations ShelterAccess Housing System Foundations Health Care 9 Prevention focuses on building a coordinated prevention network to prevent people from becoming homeless and prevent returns to homelessness among people who have previously experienced it. Create a Robust Prevention Network with Cross-System Engagement Prevent New Entries Into Homelessness Prevent Returns to Homelessness •Develop a countywide targeting strategy for prevention services •Tailor prevention approaches to high-risk populations •Streamline access and delivery of financial assistance •Align shelter operating models and funding with housing placement- focused practice •Expand eviction prevention and legal supports for formerly homeless households •Implement a systemwide progressive engagement protocol to prevent housing loss for households in RRH •Establish transfer and relocation protocols for households whose housing is not working •Develop a comprehensive plan to prevent and address youth homelessness. •Strengthen income, employment, and workforce development supports. •Create a Prevention Team within H&H to lead the implementation of the Prevention Pillar and Prevention Framework •Establish a Homelessness Prevention Network to make prevention services easier to access, streamline application processes, and support consistent information-sharing. •Strengthen partnerships with systems that serve households at risk of homelessness.
