Home Together 2030 is a refresh of Home Together 2026
Since the launch of the HT2026 Community Plan much has
been learned, and political, economic and community
landscapes have changed.
7
HT2030 envisions a system that moves people from crisis to
stable housing quickly and for the long term
•It is a framework for coordinated, strategic action – not a spending
plan or implementation handbook
•It focuses on the homelessness response system, while
acknowledging other systems and economic and political forces
that impact the homelessness response system’s success
•It sets goals that are achievable with sustained local investment,
stable federal and state partnership, coordination across
jurisdictions, and balanced implementation across pillars
➢Reduce overall homelessness by one-third
➢Reduce unsheltered homelessness by 50%
Pillar 5: Health Care

the Health Care pillar?

HT2030 is organized around five interdependent pillars,
supported by cross-cutting system foundations

System Foundations
Health Care
10
Health care focuses on improving access to low-barrier health care, behavioral
health, substance use treatment, and care coordination so health needs do not
become barriers to safety, housing, or long-term stability.
Expand Low-Barrier, Integrated Health
Care for People Experiencing
Homelessness
•Strengthen street medicine and
mobile health services.
•Expand homeless-specific
clinics and low-barrier health
access points.
•Improve access to medications
and pharmacy services.
•Strengthen ongoing care for
serious health conditions.
•Expand mobile dental services in
shelter and community settings.
Strengthen Behavioral Health and
Substance Use Supports in Street and
Shelter Settings
•Increase access to psychiatry
and mental health care for
people on the street and in
shelters.
•Expand low-barrier substance
use treatment and recovery
supports in street and shelter
settings.
•Strengthen overdose prevention
and response
•Align behavioral health and
homelessness resources.
Expand Shelter and Housing Supports
for People who are Aging and
Medically Frail
•Expand nursing, personal care,
and ADLs support in shelter and
PSH.
•Expand medically supportive
shelter models.
•Expand Medically Frail PSH
inventory
•Develop clearer criteria and
pathways for higher-support
housing and service models.
11
Improve Health Care Coordination, Transitions of Care, and Emergency Response for People
Experiencing Homelessness.
•Strengthen coordination between hospitals, emergency departments, shelters, street medicine,
EMS, and housing providers.
•Improve transitions of care for people leaving hospitals, emergency departments, and other
institutional settings.
•Support expansion of medical respite capacity for people experiencing homelessness.
•Ensure continuity of health coverage and health-related benefits for people experiencing
homelessness.
•Coordinate infectious disease prevention, testing, treatment, and follow-up in shelters,
encampments, and community settings.
•Strengthen health response protocols for encampment closures, extreme weather, and other
emergency events.
•Continue to track premature deaths among people experiencing homelessness to guide policies
and programming.
Health care focuses on improving access to low-barrier health care, behavioral
health, substance use treatment, and care coordination so health needs do not
become barriers to safety, housing, or long-term stability.
