In addition, the local health officer (or designee – the Public Health Laboratory Director) shall be notified in writing of any changes to occur in locations, dates, or times indicated in the documentation. The health officer (or designee – the Public Health Laboratory Director) shall also be notified within 24 hours prior to the program operation at the different locations, dates, or times. The law also provides for the collection of fees for the filing of documentation and related enforcement activities. The fee for the certificate is $100/year. We do not charge for additional sites. A new application shall be submitted January 1 of each year with the appropriate filing fee. It is required that the fee and a current application be on file in our office prior to a program being offered. Programs offered prior to approval of an application and fees being received will be subject to enforcement actions by the Office of the Health Officer. A certificate verifying your registration and compliance will be sent to you upon receipt of your application and fee. The certificate is valid through December 31. Please complete the attached application and return it to our office, along with your check made out to Alameda County Public Health Laboratory.
